NOTICE: The registration form(s) will be sent automatically to the CIT Program Office when using the "Submit by Email" button on the form.

To register for this program, complete the registration form and submit by choosing one of the following:

Class Registration Form
To register for any schedule class

Test Only
To register for written and performance test without a class

Written Retake
To Register when either a written or performance exam retake is necessary

Written Recertification Registration
For anyone renewing certifications

Inspection Recertification/Quiz Out and Math
Use this form to:
• Recertify any inspection certification
• Quiz out of inspection class by taking exams
• Register for math exam
