Registration

To register, either e-mail or fax the forms.

• E-mail
• Complete, necessary PDF forms listed above.
• Choose the Submit by E-mail button on the form.

OR

• Fax
• Complete, necessary PDF forms listed above.
• Choose the Print button on the form.
• Once printed, fax to 785-826-2632

Registration Confirmation
All registrations will be confirmed by e-mail.  The confirmation letter will include a map and other pertinent information.  Go to the General Information Page for more registration related information.

Program Fees
The registration fee includes all program materials, refreshment breaks, use of laboratory equipment during class, a parking permit (which must be returned) and the books/manuals unless otherwise noted.  Payment must be received before the beginning of any class or a copy of the purchase order faxed with the registration form.  Participants will not receive a KDOT certification card until all accounts are paid in full.  All registrations are on a first come-first served basis.
